Ms. Elizabeth F. Leopold – FDOT District One GBF Engineering, Inc. (GBF), a multi-disciplined construction management firm, is excited about the opportunity to provide quality CEI services to District 1. GBF has a history of meeting scheduled milestones and is committed to staying on top of the project issues and tasks to ensure that these contracts are delivered within the budget and schedule requirements.

Awareness of/Approach to Project Understanding of the scope of services

The contract entails the Construction, Engineering, and Inspection (CEI) services, which are required for the contract administration, inspection, and materials sampling and testing, for either the D/W CEI Support Contract A or Contract C. The purpose of this agreement is to enter into contract with a consultant firm to provide inspection, administrative, and surveying support on an unknown variety of projects in scope and size. As of recently, the work program has been extremely fluid, and these Contracts will ensure that last minute assignments can be handled immediately and effectively. GBF understands that assignments/task orders will vary in nature and complexity, and as such we have assembled a team with the depth of resources to provide personnel for multiple concurrent assignments. Our proposed personnel are experienced professionals with significant experience in their fields and knowledge of District 1 policies and procedures.

GBF has partnered with the leading CEI firms in the industry, experienced with working in District 1, to provide the Department with an extensive supply of available personnel and a depth of additional resources to meet any need that may arise on these contracts. The major difference between this agreement and project specific agreements are the unknowns and well defined scope of work. With a project specific agreement, many times the project documents are available well in advance of pursuing the Contract, and therefore much time can be spent reviewing those documents, speaking with Production and Construction staff, etc. and knowing exactly what will be needed to suitably staff the project. This particular agreement will see a variety of last minute assignments, with minimal time to prepare. It is imperative that a broad range of qualifications with appropriate depth is assembled to overcome this challenge. In addition, assignments will most likely be geographically located throughout the large area that District 1 encompasses. Therefore, we have teamed with several firms that we know can collectively cover all Operations Centers in the District. Our proposed team comprising of 17 consultants will provide a large pool of redundant resources for the positions listed in the Scope of Services. This will allow our Team to overcome the concern of multiple short notice, short duration assignments.

GBF and its partners have performed successfully on similar continuing/districtwide contracts throughout Florida and specifically in District 1. We have a thorough understanding of the District’s policies and procedures, share the Department’s goals on time and money and are committed to delivering a quality product within schedule and budget.

Unique Project Issues

Our understanding of the task authorization is as follows. The managing DOT office will typically request GBF to prepare a Task Work Order (TWO) for a given position for XX amount of hours or XX months. We in turn prepare the TWO along with a backup spreadsheet detailing the hours and our multipliers. Provided in that

Contract No. 11123; FM Nos: 19828616208; 40999016205 package are the following documents: Task Work Order, Backup Spreadsheet, Action Request Form, Resumes, CTQP Printouts (when applicable) and a Rate Table from CITS as back up for hourly rate. We will provide a monthly spreadsheet to the DOT Project Manager which details monthly invoices, balances and amendments for each Task Work Order. When Task Work Orders are completed, any funds remaining on that TWO are reconciled back into the Contract and can be used on new/ future Task Work Orders within that contract.

Client satisfaction is also the cornerstone of our value added approach/practices. GBF’s management philosophy is key in maintaining a high level of service for our clients. The following describes five essential elements, which will ensure the successful completion of this project: •Assemble a well-experienced, resource-rich team with relevant FDOT experience •Provide a proven staff that consistently delivers quality projects ahead of schedule •Commit to partnering, claim avoidance and issue resolution with all stakeholders •Maintain coordination with business owners and utility contacts during construction •Compose a team with members who have local presence throughout the District 1 territory and are cognizant of FDOT/District 1 policies and procedures

The invoicing process will be handled through the following steps: 1.Collect time sheets weekly after they have been approved and entered in our Accounting Software. 2.Run time by name by job report from Accounting Software for the invoice period which normally is the last Saturday of every month with a few exceptions. Verify timesheet hours with report. 3.Verify each employee’s name, position, rate and overtime eligibility as specified in the contract, task work order or action request form. 4.If there is a Sub Contractor’s invoice, check the hour summary against their timesheet hours. 5.Make sure the invoice number is in sequence and the period date should start after the last invoice date. Select the applicable FIN and schedules. Print each schedule used, summary and main invoice. Scan all CITS invoice pages and time sheets. Name PDF file by the contract number and invoice number i.e. C8K58 Inv#8. If the file exceeds 2MB, zip and send by email to the FDOT Project Manager, Project Engineer and GBF’s Management, as required. 6.Save file in the Project Invoice folder by contract on the GBF’s network/S: drive. 7.Update contract spreadsheet with actual invoices to date and remaining balance. 8.Key invoice in Accounting Software with the exact information from the CITS invoice.

Proposed Staffing

Our primary goal is to provide District 1 with experienced, available and certified personnel that will satisfy the Department’s every need, immediately, effectively and efficiently. Because these types of contracts require an ability to provide quick response and to provide services on multiple, concurrent assignments, we have assembled a resource-rich team with cross trained individuals that can fulfill the various assignments that may arise. GBF has assembled a multi-disciplined team of 17 consultants with well established CEI departments that have extensively performed FDOT work in District 1 and throughout Florida. As depicted on the resumes of our team’s proposed personnel, these firms have successfully provided CEI and related services on similar General Assign and Districtwide contracts. We have also included as part of our team AIM, DRMP and McKim & Creed for any surveying services; Tierra, Inc. for geotechnical services; GCI, Inc. for building inspection and geotechnical services; ValErin and Cella for public information services; Omni for public information and utility coordination/relocation services and HDRCC and PBS&J for any anticipated landscape architecture services.

GBF stands out above the rest for this particular contract because of our vast experience in managing Districtwide and General Assign Contracts, combined with our extensive experience in District 1. As with all our projects, we emphasize the importance of exceeding the goals of time and money, insist on high quality deliverables, and maintain error-free and accurate records while ensuring a safe environment for the traveling public and workers. GBF also ensures timely completion of all projects and client/stakeholder satisfaction. In order to provide a timely response to the Department for assignments, GBF has assigned a single point of contact for this Contract, Aslan Dilmaghani, P.E., who will be the GBF Project Manager. He will be the point of contact for both the individual task managers as well as the agreement manager. This will ensure that the information between these parties will not be lost in translation, thus saving precious time in providing staff to the Department. In addition, the GBF PM will have one point of contact for each subconsultant in order to minimize confusion between multiple contacts of a single firm when requesting information. This will make sure that information flows through one channel only, and thus in the most effective manner for quick turnarounds.

Commitment to Satisfy Department’s Need

The Department of Transportation is our “Number One Client.” Over 95 Percent of our business is directly or indirectly related to Department Specifications. GBF understands that the Department is our client. As our client, and following the faithful rule that, “the client is always right”, we consistently perform as a dependable and trusted extension of the Department. That endeavor is not just dependent on the time we are managing your contract, but extends to the time before and after the contact. We have a couple examples in District 1 specifically, where services were not provided to the Department’s satisfaction. Upon notification from the Department, GBF immediately provided the necessary forces to follow up and resolve those outstanding issues. We may not get everything right 100% of the time, and in those cases, we will drop everything to ensure that any problem is corrected and the necessary changes are made. WORK HISTORY Dyer, Riddle, Mills & Precourt, Inc. (November 2004 to present) Ms. Rupert was the Project Engineer on the $8.9-million sinkhole remediation and monitoring project on SR400 at Maitland Boulevard. This contract consisted of widening of 1-4 east bound at Maitland Boulevard, drainage improvements, construction of two new ponds, backfill and pressure grouting of a sinkhole, and construction of surcharge embankment contained by a MSE wall. Other work included installation of microwave radar vehicle detectors and highway lighting. Concurrently, she was the Project Engineer and Contract Support Specialist for the SR400 at SR423 bridge painting contract ($988,000) and the Polk Expansion of the Central Florida ITS on SR400 ($1.5-million). Ms. Rupert served as the Project Engineer and Contract Support Specialist for the $962,000.00 Central Florida Parkway Bridge Painting project. The project consisted of: cleaning and repainting the exterior surfaces of all structural steel members, installing maintenance lighting inside both steel box girders, providing timer switches at each box girder access door and other miscellaneous works on bridge no. 750402 carrying Central Florida Parkway over 1-4 (SR 400) in Orange County. In addition, Ms. Rupert acted as Project Engineer and Contract Support Specialist for the 1-4 Lake Mary Boulevard widening in Seminole County, the SR400 & SR423 Bridge Joint Replacement in Osceola County, and the SR400 Sign Structure Replacement in Orange County. This project included construction of ITS component parts along three (3) sections of the Turnpike Enterprise including SR 417 in Seminole County, SR 417 in Osceola County, and SR 429 in Orange County. Work included the installation of fiber optic communication cable, full CCTV coverage, ISMS signs, highway advisory radio, conduit infrastructure and electrical power services. This fiber communication and CCTV coverage is a part of the overall Turnpike SunNav Intelligent Transportation System.

Reference: Juan Kuthy (954) 325-1 141 Ms. Rupert served as a Roadway Inspector on Section 2A of the SR 429, Turnpike Western Expressway - Part C in Osceola County (Sand Hill Road to US 192). Work included new construction with clearing and grubbing, roadway excavation and embankment, drainage, pond excavation, stabilization, base, superpave asphalt, MSE wall construction and bridge construction.
