The Haven Hospice Mission Haven Hospice serves people and their communities affected by life-limiting illness and loss by providing comprehensive, compassionate care, while respecting each person’s needs, beliefs and wishes.

At Haven, we believe that every day is a gift. As experts in relieving pain and symptoms, as well as providing emotional and spiritual support, our goal is to help our patients and their families focus on living. We are dedicated to providing the care and support that meets the needs of our patients and their families.

At Haven, we believe that everyone deserves compassion, care and comfort. The generosity of your support helps us bring comfort before, during and after a painful loss. You help us to provide patient care to those who are not covered by insurance. You help us to offer counseling and programs to support grieving families and provide assistance to seriously ill patients who might otherwise be on their own. You help us to teach families how to care for their loved ones and educate healthcare professionals about the benefits of end-of-life care. Each of our programs is available to all who need them, regardless of their ability to pay.

At Haven, we believe that our staff and volunteers have a personal calling to share life’s journeys. Haven’s staff and volunteers go above and beyond to care for families in 18 North Florida counties from our offices in Chiefland, DeLand, Gainesville, Jacksonville, Lake City, Palatka and St. Augustine. As Haven supporters, you share their calling to help provide extraordinary support during a time of extraordinary need. During the 32 years we’ve been caring for individuals and their families, we’ve answered that call by providing a multitude of programs and services to support those we serve each and every day. PRIVILEGED TO SERVE In 2010, Haven provided direct care to more than 4,700 individuals facing life-limiting illness and supported their families through this process. In addition, we provided counseling and grief support services to more than 2,400 individuals and families in the communities we serve. • We provided significant relief of pain and other symptoms. • We gave our patients’ families the support and resources they needed. • We provided counseling and support groups for grieving individuals and families. • We served children who experienced or needed to prepare for the loss of a loved one through our Camp Safe Haven and SASHA’s Friends. • We provided assistance to seriously ill patients and their caregivers through our Haven Transitions program. • We educated health-care professionals about the benefits of end-of-life and Pallaitive Care. • We improved access to hospice care for those in rural communities, veterans, minorities and others who may not be aware of their hospice benefit. • We participated in research to foster excellence and innovation in the care we provide. • We staffed our teams with physicians, nurses, licensed massage therapists, pharmacists and other health-care professionals who specialize in end-of life care.

COMMITMENT TO THE COMMUNITY As a community-based, not-for-profit hospice serving North Florida since 1979, Haven Hospice serves 18 counties over 13,000 square miles. Haven has always been committed to providing our programs and services regardless of a patient’s ability to pay. Many of the communities we serve are largely rural and the families we care for are struggling to make ends meet. One hundred percent of the gifts and donations made to Haven remains in our communities and helps provide care for patients unable to pay, as well as provide the many unfunded programs and services we offer. In 2010, Haven spent $2.8 million on unreimbursed patient care, family programs and services, community and health professional education, and other community support.

Commitment To The Community

Patient and Family Care

At Haven, we believe that everyone deserves companionship through life’s journeys. Haven’s dedicated staff and volunteers ensure that our patients and families have support, comfort and dignity each day. By serving them in the setting where they are most comfortable, and providing relief from pain and symptoms, our patient and family care offers them choices and honors their wishes. In 2010, your donations helped offset $1,463,308 in unreimbursed patient care expenses.

Healing Hearts

Our grief support services are available at no cost to our patients and their families. We offer them above and beyond our patient services to enhance the care we provide. We also make these services available to anyone who has experienced a loss, whether or not they have been served by Haven Hospice. For many in the community, Healing Hearts is their first experience with Haven’s compassion, care and comfort. To honor the lives of family, friends and loved ones, we host Love and Remembrance services in May and November. Last year, 2,400 individuals benefitted from the services provided by Haven’s Healing Hearts program. Your gifts in 2010 helped support $87,659 in unreimbursed expenses for bereavement services and children’s programs. This amount does not include the cost of our Chaplains and Social Workers who spend thousands of hours counseling our patients and their families. 7

Commitment To The Community

Haven Transitions Haven Transitions provides education and support that can make a world of difference for someone facing a serious illness, including supportive phone calls and volunteer assistance from those who can benefit from companion visits. On average, the program serves more than 320 clients and their caregivers every day. Transitions’ community education programs help people interested in learning more about living with a chronic illness, preparing advance directives, and so much more. Transitions provided services to 839 individuals and their families last year. Your donations in 2010 helped to offset more than $67,793 in unreimbursed expenses for Haven Transitions.

Children’s Programs:

Camp Safe Haven and SASHA’s Friends A grieving family deserves unconditional support from those who understand that no loss is like another, and no one grieves in the same way. We believe that nothing is more important than providing this support to children who have recently experienced a loss or are anticipating a loss. Our grief support services include Camp Safe Haven, for children who have lost a loved one, and SASHA’s Friends, a support group for kids who are preparing to lose someone close to them. Each year, Haven’s Healing Hearts hosts Camp Safe Haven for children, ages 6 – 12, and for teens, ages 13 - 17. Supported by Haven staff and volunteers, these children participate in fun and therapeutic activities to help them work through their grief. They also find comfort in sharing their loss with their peers who are also grieving. Camp Safe Haven and SASHA’s Friends served more than 125 children in 2010.

Palliative Massage Therapy Gentle, compassionate touch by Haven’s specially trained licensed massage therapists enhances our patient care by helping patients relax; relieving their pain, stress and anxiety; increasing their physical comfort; and improving sleep. Last year we were able to provide more than 6,619 palliative massage treatments for our patients. Your support of Haven in 2010 helped offset $281,856 in unreimbursed expenses for this program. 9

Commitment To The Community Volunteer Services

Haven volunteers not only enhance the lives of our patients and their families, but also add meaning to their own lives. Our specially-trained volunteers work closely with our patient care teams to assist in providing much needed support to patients and families. They also reach out to grieving families, help staff our Attic Resale stores and serve on our special event committees. Your 2010 donations helped offset $465,246 in unreimbursed expenses required to recruit, educate and train volunteers, in addition to the community outreach performed by these dedicated individuals.

Community & Health Professional Education Haven Hospice’s commitment to the community extends beyond providing hands-on clinical, emotional and spiritual care. Haven is also committed to providing information and education about living wills and advance directives, coping with grief, caring for the caregiver, and much more. In addition to community education, Haven provides clinical education programs to physicians, nurses, social workers and nursing assistants in area hospitals, long-term care facilities, and doctor’s offices to help them better understand and appreciate the unique issues surrounding end-oflife and palliative care. Additionally, we train them in helping to care for a hospice patient while they are in their facilities.

Haven Hospice also provided clinical education to future health professionals from area colleges, universities, and community colleges by providing students with internships, clinical rotations, course instruction and lectures. Your support of Haven in 2010 helped offset $428,901 in unreimbursed expenses for this program.

A Haven Care Center for Clay County This past year, Haven applied for a Certificate of Need to build our new care center in Clay County. We received approval from the Agency for Health Care Administration in August 2010 to build an 18-bed inpatient care center in Orange Park, which will serve the residents of Clay County. More than 180 physicians, health care administrators, community services managers, and many others in Clay County supported our Certificate of Need application with letters of support. We are thankful for their heartfelt support of this initiative and look forward to providing inpatient hospice services in Clay County in the coming year. The communities we serve also supported Haven as we’ve built inpatient care centers in Chiefland, Gainesville, Lake City and Palatka. Each provides specialized care in the communities we serve and is conveniently located so that patients and families can access them without having to travel a distance or forego inpatient hospice care altogether. A Haven care center is equipped to cater to short-term inpatient needs and, therefore, offers families an option other than admission to a hospital. It also provides respite care support for families and caregivers who need a break. We estimate that we will invest more than $7 million in this new care center. The project represents more than 18,000 square feet of new construction, including the land, building and equipment. We expect to employ 23 full-time employees who live and work in Orange Park and its neighboring communities. They will include registered nurses, licensed practical nurses, nurses’ aides, an administrator, secretary, medical director, social worker, cooks and housekeepers. As we prepare to build and open our doors at this newest care center in Orange Park, we invite you to learn about ways you can support this project. There will be naming opportunities for the building and patient rooms, and our brick memorial walkways and stone benches provide a unique way to remember your loved ones. THE COMMUNITIES’ SUPPORT In 2010, donors to Haven Hospice gave gifts in honor or memory of a loved one; attended or sponsored a special event; responded to an annual appeal such as Light Up a Life or ForgetMe-Not; donated items to or shopped at our Haven Attic resale stores; or remembered Haven in their wills. In all, their generous support provided more than $1,571,752 in support. These donations are vital to Haven and allow us to provide many programs and services to our patients and their families that otherwise would not be available. The Memorial Walkways are dedicated to your loved ones. Walkway donors create a permanent legacy. Bricks, granite pieces and benches along the Memorial Walkways are inscribed to honor loved ones according to your wishes.

Haven supporters embrace the opportunity to uphold our mission through fundraising and volunteering. Many support our events through their attendance and participation, and increasingly, organizations in the communities we serve are offering to host special events that benefit Haven. In 2010, these special events throughout North Florida and those who sponsored them raised $280,967 in cash to benefit Haven.

In 2010 Haven Hospice received $114,047 through our spring Forget-Me-Not appeal, our fall Light Up a Life appeal, our In Touch newsletter, and in response to our Annual Community Report.

In 2010, Haven received more than $327,470 in gifts and pledges in honor or memory of a loved one. These donations serve as a primary source of support for unfunded and unreimbursed programs and services offered by Haven. Many families and friends request that donations be made to Haven Hospice in lieu of flowers, or they wish to memorialize their loved ones along the Memorial Walkways located at our care centers.

The Community’s Support Planned giving enables donors to arrange charitable contributions to Haven Hospice in ways that maximize their personal objectives while minimizing the after-tax cost. Depending on the asset given to Haven Hospice and the gift arrangement selected, a donor can generally expect to obtain some or all of the following benefits: • Fulfill philanthropic goals • Reduce income tax through a charitable deduction • Avoid capital-gains tax on gifts of long-term appreciated property • Retain a stream of income for life for yourself and/or other beneficiaries • Increase spendable income • Eliminate potential federal estate tax on property that passes to Haven Hospice • Reduce costs and time in estate settlement

Planned Giving

Planned giving is a powerful way to provide major support for Haven Hospice. In 2010, Haven received $21,879 from donors who named Haven Hospice in their wills, estates or other planned giving vehicles. These donations are crucial to the patients and families who benefit from the many unfunded programs and services Haven Hospice offers.

The Haven Hospice Endowment is a separate account of funds that provides future longterm income to Haven Hospice to help support unfunded patient care and our many community and family support programs and services. The Haven Hospice Board of Directors controls any distribution and/or reinvestment of funds associated with the endowment. As of December 31, 2010, the balance of the endowment was $1,489,469.

We appreciate our very generous donors who bring their new or gently used treasures to us. Donations are always welcome and go a long way toward helping the families and communities we serve. Sales from your donations benefit our patients and their families who are unable to pay for care, as well as those served by programs such as Camp Safe Haven for children who have experienced a loss. Countless individuals, businesses and organizations generously donate items ranging from housewares and furniture to clothing and children’s toys.

have challenging economic circumstances and need an affordable place to shop. Haven is seeking energetic, conscientious volunteers who are mindful of the Haven mission. We truly could not do what we do without our volunteers. You can also support the Attic by hosting a donation drive or bringing the Rollin’ Attic van to your place of business or neighborhood. Our Haven Attic Resale Store donation trailer, a 23-foot-long semi truck, is stationed on a rotating basis at businesses throughout the Tri-Counties area. The Attic also partners with community organizations to provide much needed clothing and housewares to the homeless, displaced homemakers, victims of fire and many others in need. We are also proud of our partnership with Kids Closet that provides new or like new coats to children in need. In December, we host Kids’ Holiday Shopping Nights to help children shop for family members who otherwise could not afford to exchange gifts. In 2010, the Attic stores generated combined net revenue of $530,002 thanks to our generous donors, patrons and volunteers.

Haven Attic volunteers make a difference by sorting donations, stocking the floor and making friends. But above all, they help people throughout the Haven Hospice community— patients and families, as well as people who

Haven Hospice Attic Store for Orange Park The newest Attic Resale Store opened in March 2011 in Orange Park. The store is located at 1801 Wells Road, just across the street from the Orange Park Mall. It features 6,000 square feet of showroom space and is filled with wonderful treasures. The store has been warmly received by the people and businesses of Orange Park and Clay County and we look forward to partnering with the community as we go forward.

Community Campaigns

In 2010, Haven Hospice received $83,190 from generous individuals throughout our communities that designated Haven as their charity of choice in their associated employee campaign. Donors to the University of Florida Community Campaign (UFCC), the Combined Federal Campaign (CFC) and the Florida State Employee Community Campaign (FSECC) have designated their gifts to Haven. Additionally, even though Haven is not a United Way agency, anyone participating in a United Way employee campaign can name Haven as a designated agency of choice.

DeLAND

Volunteers: Barbara & Harry Kirkpatrick Barbara (B.J.) and Harry Kirkpatrick are dedicated to supporting Haven patients and their families by listening and doing all they can for them. The two felt called to volunteer at Haven after they shared an experience helping an elderly couple. “A friend asked me, ‘What would have happened if you hadn’t been there for them?’” B.J. said. That got her thinking she needed to be there for others as well, and she decided to call Haven to volunteer. “We decided it would be a privilege to be involved in this part of the journey,” said Harry, her husband of more than 14 years. “It’s nice to have a part in patients’ lives, even when we lose them,” says Harry. “I don’t think someone can talk you into doing what we do. I can only say how much you get out of it. The patients, their spouses and the families we serve are so grateful for your help.”

CHIEFLAND

Volunteer: Arnold Dittenber Arnold Dittenber is a Haven ambassador, recruiter and one-man welcome committee. Whether he’s out in the community drumming up new volunteers for Haven or greeting veterans at a Veterans Day breakfast, he makes everyone around him feel appreciated. A retired hardware store owner who moved to Florida from Michigan, this longtime Haven volunteer lost his wife Shirley after a 2-1/2 year battle with cancer.

Fall at the Lodge Dedicated sponsors, volunteers, guests and a wonderful planning committee made Fall at the Lodge a beautiful evening under the stars September 18 at the Gilchrist Club in Trenton. More than 300 guests and volunteers feasted on delectable country food prepared by the Gilchrist Club chef and enjoyed clay shoots, gaming and crab racing. Guests also participated in live and silent auctions. The event generated more than $54,000 in total support to benefit the many programs and services Haven Hospice provides the community.

Badcock Furniture

Shirley was his partner in life and in volunteering at Haven together. She died the day before their 11th anniversary last year. “Volunteering at Haven means even more to me now because of how Haven cared for Shirley,” he says. The couple started with Haven during our campaign to establish the Tri-Counties Hospice Care Center, and they funded a patient room in memory of both their parents. The greatest privilege of being a volunteer, says Arnold, is that “there’s so much you can do.”

Badcock Home Furniture & More of Chiefland raised $4,597 during its fourth annual benefit for Haven Hospice—a holiday drawing for a large-screen high-definition TV—bringing its combined contributions to more than $16,000 for the organization. Badcock has been a faithful supporter of Haven Hospice ever since the organization took care of owner Carol Tew’s husband, Lamar. The annual drawing has become a local holiday event that shows Tew’s appreciation for Haven while also benefitting others.

The Community’s Support GAINESVILLE Donor: Nancy Perry

Five years after the loss of her husband, Chuck, Nancy Perry finds herself reminiscing all the time. She cherishes the last pair of shoes he wore and the wallet that held his first Boy Scout membership card. She even kept his

terms, our whole philanthropic approach to life. That really began the journey for us. On so many levels, it made such good sense to support this project all we could.” The Perrys funded a patient room and were comforted when a dear friend made her final journey there. “We thought, ‘wow, we’ve done something special,’” Nancy says. “Everyone is capable of making such a difference, whether now or through your will or trust. We were proud to do it.”

Run for Haven

Events: Tioga Town Center Run for Haven Perfect weather was the order of the day for 500 runners, walkers and party-goers who came out for the inaugural Tioga Town Center Run for Haven. Forty-five volunteers and 14 sponsors and vendors participated in the event, which raised $18,000 to support Haven’s care and services for patients and families.

York Hospice Care Center in Gainesville, “it brought us together to discuss it,” Nancy says. “We saw it as an opportunity for spiritual growth, a way of adopting, in loving

ViVA! 2010—Goes Polynesian More than 500 guests enjoyed an evening full of true Polynesian-inspired cuisine, hula dancing and a hot-air balloon at the 6th annual ViVA! 2010 – Goes Polynesian, April 10 at the Rembert Farm in Alachua. Michael Pappas, executive chef of Harry’s Seafood Bar and Grille, and his staff prepared mouth-watering Polynesian-inspired cuisine for the occasion, and the festivities featured lively music and dancing by Island Vision Productions. Guests also enjoyed a gaming tent, a cigar roller, crab racing and much more, including live and silent auctions. ViVA! 2010 raised more than $250,000 to support Haven’s patient care, services and programs.

Tenth Annual Styles For Life Fashion Show The Haven Hospice Alliance hosted the Tenth Annual Styles for Life Fashion Show November 4 at the Paramount Plaza Hotel and Conference Center in Gainesville. KTK’s Storm Roberts emceed the show, which featured Florida Senator Steve Oelrich, Alachua School Board member April Griffin and Gainesville Chamber of Commerce president Eric Godet and others modeling fashions by Belk, Coldwater Creek, Rapid City Clothiers (RCC Western Stores), Stein Mart and the Haven Hospice Attic. More than 280 guests helped the Alliance raise more than $15,000 to support Haven programs and services.

The Community’s Support JACKSONVILLE Donor: Michael Pappas

Michael Pappas, Corporate Executive Chef of Harry’s Seafood and Grille, fires up a first-rate guest experience at ViVA each year, supported by a kitchen crew and wait staff who look forward to coming back and being a part of it. “We’re all out there doing what we have to do and doing it for a great cause,” says Michael, who is delighted to donate his time to Haven. “It’s always good to be involved and giving back,” he adds. “Our goal is to make it look easy.”

Michael had no idea when he signed on for ViVA four years ago that he would be turning to Haven to care for his parents. He lost his mother in March, just before ViVA 2010, followed by his father in September. “We could not have done it without Haven,” he says. “The same day we called, Haven was there with everything we needed, and there was a calming of the whole family. The only thing that matters is time. Haven gave me quality time to spend with both my parents.”

Joes v Pros

Event: Joes v Pros Celebrity Softball Challenge Sunday, December 5 proved to be a beautiful day for a softball game, as Haven Hospice and the Jacksonville Suns hosted the inaugural Joes v Pros Celebrity Softball Challenge at the Baseball Grounds of Jacksonville. WJXT’s Bruce Hamilton emceed the event, featuring the University of North Florida’s baseball team, former Gator and Chicago Bear great Neal Anderson, and Major League Baseball pitchers Mike Jones and Joel Davis as the “pros.” The “joes” consisted of children from the Jacksonville area who were selected at random from the attendees. The festivities included baseball training stations, food, live music, a silent auction and a grand finale softball game of “Joes v Pros.” American Idol Season 6 contestant Phil Stacey was the headline performer. Randy “Elvis” Walker and a local band, Lawless Hearts, also performed.

Many of the families we serve want to thank us by giving back, and they find creative ways to do it. Last year, people in the communities we served donated proceeds from Daytona Magic, a country music concert, a haunted house, and a misfit car show. In Lake City, Ms. Wezzie’s Haircut, which donates half its income back to the community through time or treasure, hosts the annual Uncle Bob’s Cut-a-Thon to benefit Haven. This year, Ms. Wezzie, as she is called, and hairstylist Erin Misinec cut hair for more than four hours in memory of Power Country 102.1 Radio Station’s founder and general manager, “Uncle Bob,” who died in October 2008. Co-sponsored by Power Country’s “Cousin Ron” Peacock, the initiative raised $425 for Haven’s unfunded programs and services. “Haven has helped some of my family and friends and I wanted to give back to such a great cause,” says Ms. Wezzie, the shop’s owner. “Haven Hospice does so much for this community and we wanted to help them in their efforts.”

Doris R. Satterwhite taught physical education and coached the girls’ basketball team at Palatka High School (PHS) for 16 years. She donated a room at Haven’s Roberts Hospice Care Center in honor and remembrance of her former students at PHS and helped fund another room in memory of Barbara Jean Poindexter, a dear friend who was one of her best players. When Haven announced plans to build the Roberts Care Center, Doris was talking it up in the community, encouraging friends to help. She formed lasting friendships with her players, who were only a little younger than she when she began coaching. From the time this North Carolina native moved to Palatka to start her career, she saw herself as someone who could help others along the way. “There are so many people who have no caretakers or anyone at home to take care of them,” she says. “Haven’s care center gives them a place to go. They can find comfort here, and it helps their families so much.” 21

Event: NEFAR Charity Bass Tournament More than 20 volunteers and more than 80 fishermen and women participated in the Northeast Florida Association of Realtors (NEFAR) Seventh Annual Charity Bass Fishing Tournament on March 27 in Palatka. This tournament draws top fishermen and women from all over the East Coast, and continues to be the largest source of donations to support Haven Hospice services and programs in the Putnam County area. The event raised more than $15,000, presented by NEFAR CEO Glenn East and the NEFAR committee. During the last seven years, NEFAR has donated approximately $124,000 to support patients served by our Roberts Care Center.

ST. AUGUSTINE Donors: Haven Volunteers

Haven volunteers and staff are always mindful of our patients’ special needs, and they love going above and beyond to enhance the care we provide, from decorating little holiday trees for patients’ rooms to collecting food that provided Thanksgiving and Christmas meals for the neediest families we serve. In St. Augustine, Haven volunteers Karen Jakoby led an initiative to create gift baskets for each patient. Volunteers shopped, baked and made greeting cards for each basket, which featured the warmth of a handcrafted afghan or blanket. The families we serve appreciate these extra gestures. One of our patients wrote: “What a wonderful surprise, and so unexpected— a Christmas basket of goodies. I particularly like the handmade quilt with dogs and the little Snoopy with his blanket. As a veterinarian, I really miss contact with dogs. So—many, many thanks for all the love and support the whole staff has shown us during this difficult time.”
